Who qualifies?            

- Any young person under the age of 18

- Who has been doing work on tobacco control and prevention at least (if not longer!) since January 1, 2006

- Who is a Massachusetts resident

How do I apply?         

- Fill out this form and answer all the questions.

• Get someone to fill out the reference form (an adult or peer).

And then what?         

- Send it all in by Thursday, March 8, 2007 (it should arrive in our hands by that date—email, fax, postal mail, hand delivered).

- You will be notified on Saturday, March 17, 2007 at the YAI Youth Summit if you have been selected!

What’s included in the award?    

- Your photo and profile will be posted on our website.             

- You will receive a gift certificate.

- You will get to add this to your resume and college applications!

- We will send a press release announcing this award to your local papers
